How to enroll at GOVS
Enrolling is free, happens online, and takes just a few steps. Here's exactly how it works, start to finish, so you know what to expect.
Information on Registration for 2026-2027 school year
The entire application process can be completed online.
Please Note: All offices are now located in Franklin at our Student Center. The address is 24 W. 1st Street Franklin, OH 45005. Appointments must be made in advance for help with registration. Any questions, and or to make an appointment please email mindy.mcgregor@mygovs.com.
Follow these steps to enroll
Please read carefully as these steps are crucial to completing your application for registration. Failure to do so could result in a delay in your application.
Go to FinalForms

Create Your Parent Account
Click the enrollment link to open FinalForms: https://govs-oh.finalforms.com/
Add Your Student
Once logged in, add your student to your parent account.
Complete All Forms
Click Incomplete Forms. All forms must be filled out — a form is done when it turns green. Don't move to the next step until every form shows green.
Open the Document Folder
Click the folder icon to access the required enrollment documents.
Upload Required Documents
Follow the instructions to upload the following REQUIRED documents:
Documents required for registration include:
- Student's original birth certificate
- Parent/Guardian valid driver's license or photo ID
- Proof of Residency (Rent/lease agreement, mortgage statement, copy of deed, land contract or purchase contract.
- Completed and Notarized Affidavit II (based on your current living situation). It must be completed and signed in the presence of a commissioned notary. Most local banks will notarize this at no cost, but it is recommended that you confirm this ahead of time. Appointments can also be made with a notary in the Registration office. (Links to the Affidavit can be found at www.mygovs.com under Parent Resources, forms & information ).
AFFIDAVIT CLARIFICATION
Use Affidavit II if:
- You are the minor student’s parent/guardian BUT the proof of residency is in another person’s name
- You are the parent of a student over 18 who still resides with you and the proof of residency is in your name
- You and your family are living with another family (either temporarily or permanently) and the proof of residency is in another person’s name.
- Custody/Guardianship Paperwork (if applicable). Please upload the custody documentation in one acceptable document. Do not scan the custody paperwork as individual pages. We must have the COMPLETE custody or court documents. Please make sure to send ALL pages.
- Immunization records - not a registration requirement but are required before school starts.
You're Almost Done — Here's What Happens Next
Once ALL of the steps have been properly completed, you will receive a confirmation email from the registrar with further information!
Students DO NOT need to sign their forms for registration to be completed. They will receive instructions on how to do so once they have started school.
Important: don't leave your current school too soon
It's worth repeating, because it matters. Submitting your GOVS registration does not make you a GOVS student yet, and it does not mean you should stop attending your current school.
Wait until our office reaches out, confirms your paperwork, and schedules your start date. They'll tell you exactly when and how to make the switch so nothing falls through the cracks.
What happens after you enroll
Once you're officially enrolled, your counselor reviews your transcript and builds your schedule around the credits you still need. You'll have a window after your start date to ask questions or request schedule changes before your courses are locked in.
From there, you log in, start working, and your team - coaches, mentors, teachers, and tutors - is ready when you need them.
